MAILBOX SERVICE AGREEMENT
This Mailbox Service Agreement ("Agreement") is made and entered into by the customer identified above ("Customer") for the use of and services related to a mailbox ("Mailbox") at **Financial Powerhouse LLC DBA Mailbox Depot** ("Company"), under the terms set forth herein and governed by the laws of the State of Kentucky.
**Notice for College Students:** If you select the College Student Mailbox option, you must upload a valid, current student ID as part of the application process.

1. **Permitted Use**
The Customer agrees not to use the Mailbox or the Company’s premises or services for any unlawful, fraudulent, or prohibited activities, including those restricted by U.S. Postal regulations. The Mailbox use must comply with all applicable federal, state, and local laws. Each authorized recipient must complete and submit U.S. Postal Service Form 1583 ("Form 1583"). Spouses may complete a single Form 1583, provided both include their separate information.

2. **Confidentiality and Compliance**
The Agreement and Form 1583 will be kept confidential but may be disclosed as required by law, including written requests from law enforcement or governmental agencies. Customers must complete all required documents, including updates to Form 1583 or this Agreement, when requested by the Company.

3. **Access and Authorization**
Possession of mailbox login credentials shall constitute valid proof of authorization for access. Upon the Customer’s death or incapacitation, access will require proper legal documentation, such as probate or executor authorization.

4. **Termination and Handling of Mail**
Upon expiration, cancellation, or termination of the Agreement, the Company may take the following actions:
1. **Mail Forwarding:**
- Mail may be forwarded for up to six (6) months if the Customer pre-pays all postage, material, and handling fees.
- A storage fee of $10 for the first month applies; subsequent months will incur fees as determined by the Company.
2. **Mail Storage:**
- Mail may be stored for up to six (6) months at a storage fee of $20/month, plus a service fee for in-person retrieval.
3. **Mail Retention and Disposal:**
- Mail not forwarded or picked up within ten (10) days may be discarded or destroyed.
- Unsolicited mail (e.g., bulk advertisements) may be discarded immediately.
4. **Package Refusal:**
- Packages from third-party couriers may be refused unless pre-approved.
